From midlife onwards, three common vascular factors weigh heavily on your years of dementia-free life, according to a large American cohort. Hypertension, diabetes and tobacco draw a dizzying gap, from which NYU Langone researchers draw a clear warning.

In midlife, three very common health issues could mean the difference between a lucid retirement or years cut short by memory problems. A large American study published on August 5 in the journal Neurology Open Access shows that adults followed for an average of 26 years lived on average nearly 13 more years without dementia
when they had none of these vascular risk factors at midlife, compared to those who had all three.

Hypertension, diabetes and tobacco: the trio that costs 13 years without dementia

This work, carried out by the NYU Langone Health team from theAtherosclerosis Risk in Communities (ARIC) Studyfollowed 12,409 people with an average age of 56 at baseline, all without dementia. The researchers were interested in three simple markers to check during consultation:
blood pressure
THE diabetes and the
smoking.

In ARIC, participants were categorized by the number of factors present in their 40s and early 50s: none, one, two, or three. Those without hypertension, without diabetes and non-smokers lived on average 30 years after the start of follow-up without developing dementiacompared to 17 years for those who combined all three risks. In other words, this trio is associated with a loss of approximately 13 years of life with a still functional brain.

The team also observed significant differences between groups. Among people with all three factors, women lived an average of 18.1 years without dementia after the start of follow-up, compared to 16.6 years for men. White participants in this same situation totaled 19.6 dementia-free years, compared to 16.0 years for black participants.

As Josef Coresh, director of the Optimal Aging Institute, summarizes, “our results indicate that people should actively avoid these factors in midlife as a strategy to preserve brain health for more than a decade“.

Why midlife is a pivotal time for the brain

The authors of the article in Neurology Open Access
point out that hypertension, diabetes and smoking often appear in mid-life and have lasting effects on the mechanisms that lead to neurodegeneration. These factors gradually damage the small vessels of the brain, reduce blood supply, maintain inflammation and weaken the barrier that protects brain tissue. Over several decades, this wear and tear makes the brain less resistant to the lesions responsible for dementia.

For Josef Coresh, the issue goes beyond simple individual comfort given the aging of populations and the absence of therapeutic solutions. The key idea of ​​the study is to speak in “years of life without dementia”, a measure more meaningful for patients than an abstract risk percentage.

What the study changes (and what it doesn’t say) for your midlife

The authors remain cautious: the study was not designed to prove that avoiding these three factors explains the 13-year gain, only that the two are associated. The factors were measured only once, without tracking changes in treatments or habits over time, which may even underestimate the true impact. But for researchers, the message is already clear: “These findings will hopefully encourage people to quit smoking and closely monitor their vascular health starting at age 48“, emphasizes Josef Coresh.

In practice, this means taking advantage of your forties and fifties to have your blood pressure checked regularly, screen for possible diabetes and seek help to quit smoking. The study shows that it is not just a matter of protecting the heart or the arteries: these three actions could also postpone the onset of possible dementia by several years, in a period of life where each year of cognitive autonomy counts.
