
Doctors point out that double vision that appears suddenly should always be taken seriously. It can reveal a stroke in progress. Understanding why this happens and learning to recognize other warning signs can literally save lives.
When the eyes stop working together
We’ve all experienced that moment when our vision gets a little blurry, after a long day or lack of sleep. In most cases, it is only temporary fatigue. But when you suddenly start seeing double, caution must be exercised.
This perception of two images for a single object has a medical name: diplopia (double vision). And contrary to popular belief, it is not always a simple eye problem.
“Double vision that appears suddenly is a medical emergency.”insists Dr Gérald Kierzek, emergency doctor. “This symptom may indicate a stroke in progress and requires immediate treatment to limit damage to the brain.”.
To understand this phenomenon, we must remember that our eyes function as a perfectly synchronized duo. Each captures a slightly different image, which the brain merges to create a unique and stable perception of the world.
But during a stroke — whether an ischemic stroke caused by a clot or a hemorrhagic stroke — certain regions of the brain can be affected. Among them, those which control the nerves responsible for eye movements.
“The vascular accident can affect the nuclei of several cranial nerves located in the brainstem.explains Dr. Kierzek.
These nerves control the muscles that direct the eyes. When disrupted, the precise alignment of the two eyeballs breaks. The eyes no longer look in exactly the same direction. The brain then receives two different images that it can no longer merge.
The result is immediate: the vision is duplicated. Depending on the affected nerve, the images may appear side by side or on top of each other.
Visual signals that should alert you
Double vision linked to a stroke rarely appears in a trivial context. It is often accompanied by other visible anomalies, sometimes discreet but very evocative.
For example :
- An eyelid that suddenly droops (ptosis)
- An abnormally dilated pupil;
- The sudden appearance of strabismus.
A clue can also help to understand what is happening: in diplopia of neurological origin, vision generally returns to normal when one eye is closed. This detail indicates that the problem does not come from the eye itself but from the misalignment between the two eyes.
These signs may seem impressive. However, doctors point out that rapid treatment can sometimes allow recovery. In other words, time plays a decisive role.
Stroke: recognizing the emergency to save the brain
Double vision should be even more alarming if it occurs with other neurological symptoms. To quickly identify them, healthcare professionals use the rule
QUICKLYthe French equivalent of the Anglo-Saxon benchmark FAST.
It is based on four simple signals:
- V for Face : the smile becomes asymmetrical;
- I for Incapacity : difficulty raising an arm or weakness on one side of the body;
- T for Speech Impediment : difficulty speaking or confused speech;
- E for Extreme Emergency : Emergency services must be called immediately.
Other symptoms may also appear: numbness on one side of the body, sudden dizziness, loss of balance, severe headache, confusion or nausea.
Even isolated, double vision which appears suddenly – or which persists for more than an hour – should prompt you to consult without delay.
“If double vision appears suddenly, you must immediately call 15, 18 or 112.recalls Dr. Gérald Kierzek. “And above all, don’t drive yourself”.
If you have a confirmed stroke, doctors may sometimes use a treatment called thrombolysis. Its objective: to dissolve the clot which blocks blood circulation in the brain. But this treatment is extremely time-dependent. It must be administered within 4:30 a.m. following the onset of symptoms, and ideally within first three hours.
Every minute that passes, thousands of brain cells disappear.
Behind this simple visual symptom there is sometimes a real race against time. Recognizing this warning sign, not trivializing it, can make all the difference. For doctors, the message is clear: faced with sudden double vision, it is better to consult unnecessarily… than to regret having waited.
